What is included
- Basic Tele-Health: video and phone visits, messaging through the portal and WhatsApp, prescriptions sent electronically, discounted labs and a yearly preventive-planning visit.
- Standard Hybrid: everything in Basic, plus up to two in-person visits a month, priority scheduling, a quarterly check-in and portal refills.
- Family / Group: care for up to four household members, telehealth for everyone, in-person visits when needed, school and camp physicals, and a monthly wellness webinar.
- Premium Concierge: everything in Standard Hybrid, more in-person access, up to two home visits a month in our service area, and a personal health-plan review.
What costs extra

- Imaging, specialist visits and hospital care (through your insurance)

The yearly math
Pay per visit is the lowest-cost way to see us if you only need care now and then: four visits a year start at $340. A Basic Tele-Health membership is $1,020 a year and is about access rather than the cheapest visit: messaging between visits, priority scheduling, a yearly preventive-planning visit and a clinician who follows up with you. Is there a sign-up fee?
No. You pay the monthly membership, and every plan comes with a 30-day trial.
Can I cancel?
Yes, with 48 hours’ notice. During your first 30 days, cancellation and any refund are discussed with your provider.
Can I change plans?
Yes. Plan changes are made after a short consultation with your provider so the new plan fits your care.
