How to Use Hydrocortisone and Steroid Creams Safely

Get the most from steroid creams: how much to use, how long, where to be careful and when a rash needs a different treatment.

WheelHouse Health guide to safe use of topical steroid creams

Strengths explained

Topical steroids range from mild (hydrocortisone 1%) to very potent (such as clobetasol). Stronger creams work faster for thick skin and stubborn rashes but carry more risk on thin skin. The face, eyelids, armpits and groin need the mildest options.

How to apply

  1. Wash and dry your hands.
  2. Squeeze out a line of cream from the fingertip to the first crease: one fingertip unit.
  3. That amount covers an area about the size of two adult palms.
  4. Apply a thin layer to the rash, gently, once or twice daily.
  5. Use moisturizer separately, ideally 15 to 30 minutes apart.

Cautions

  • Do not use for more than about 1 to 2 weeks without advice.
  • Avoid on infected skin, cold sores, acne and fungal rashes (unless prescribed).
  • Long-term use can thin skin, cause stretch marks and lighten skin color.
  • Stop gradually after long use to avoid a rebound flare.

Questions people ask

Can I use hydrocortisone on my face?

Mild hydrocortisone for a short time is usually fine. Avoid the eyelids and do not use it for acne or rosacea.

Can steroid creams make a rash worse?

Yes, if the rash is fungal, it can spread. That is why an uncertain rash should be checked.

Is it safe for children?

Mild steroids used briefly are usually safe. Ask before using on babies or for long periods.
