Many practices have a patient portal where you can download a summary of care. Share it with us through our patient portal.

How we can help
We can send the request on your behalf once you sign a release. Bring whatever you have to your first visit; we will start care either way. See switching your primary care provider.
Questions people ask
How long does a records transfer take?
Often one to two weeks, and providers must respond within 30 days.
Can my old provider charge me?
They may charge a reasonable, cost-based fee for copies under HIPAA.
Do I need my records before my first visit?
No. They help, but we can start without them.
